Our DVA Services
- Personalized Care: Our clinical nurses works with each veteran to create a customized care plan that prioritizes their physical and emotional health, promoting independence while offering support where needed.
- Supported Independent Living (SIL): We provide SIL services that allow veterans to live independently in their own homes or community-based housing, while receiving tailored assistance to maintain their daily routines.
- Respite Care: For veterans and their families, our respite care services provide relief from caregiving duties, ensuring both veterans and their carers have time to recharge.
- Mental Health Support: We understand the mental health challenges that many veterans face. We offer specialized support to help with PTSD, depression, anxiety, and other mental health conditions.

Community Nursing
Eligibility for DVA-Funded Services with OptimumCare Plus
To be eligible to receive care services for an assessed clinical need with OptimumCare Plus, you must hold a Gold or White DVA Health Card. If you meet the eligibility criteria, there will be no cost to you, as the Department of Veterans’ Affairs (DVA) will cover the cost of services provided by OptmumCare Plus.
