OptimumCare Plus Support coordination is a valuable service that enables you to maximize the effectiveness of your plan. It is a capacity-building support that assists you in:
In summary, there are three levels of support coordination available to be included in your plan:
1. **Support Connection:** This level focuses on enhancing your ability to connect with informal, community, and funded supports, enabling you to maximize your plan and work towards your goals.
2. **Support Coordination - Coordination of Supports:** This support helps you develop the skills required to understand and utilize your plan. A support coordinator collaborates with you to ensure a diverse range of supports is used, thereby increasing your capacity to maintain relationships, manage service delivery tasks, live more independently, and actively participate in your community.
3. **Specialist Support Coordination:** This represents a higher level of support coordination, specifically designed for individuals with more complex situations requiring specialist assistance. A Specialist Support Coordinator is there to help you manage challenges within your support environment and ensure the consistent delivery of services.
The services within support coordination encompass various aspects, including:
1. Coordinating and overseeing services for individuals with complex needs.
2. Conducting one-on-one meetings to discuss and clarify the plan.
3. Aiding participants in optimizing the value derived from their plan.
4. Assisting in the setup of the NDIS portal.
5. Offering guidance on understanding service agreements.
6. Conducting research and suggesting service providers tailored to individual requirements.
7. Providing support in reaching out to service providers.
8. Offering ongoing support even after a participant's plan has been put into action.
9. Preparing both the participant and the necessary paperwork for plan reviews.
Our Commitments:
1. Delivering a team-based approach, ensuring that if one team member is unavailable, another will step in to assist participants.
2. Assisting participants in learning how to coordinate their needs if they choose to do so.
3. Ensuring that experienced professionals in mental health, intellectual disability, positive behavior support, and restrictive practices are available at the required level.
4. Managing any potential conflicts of interest and recommending service providers that best match participants' needs.
OptimumCare Plus will employ certain categories of allied health professionals to work as Specialist support coordinators:
§ Psychologist
§ Psychosocial and Recovery coaches
§ Occupational Therapist
§ Social Worker
Other allied health, developmental educator, social or health science professionals.
